360 how to report?

i have someone that wont stop bothing me i blocked him but he finds other ways to do this, how do i report him on 360 ? i know to click report but there is an eara in the page that i dont understand. it ask for something like a url or addy im not sure what that is and how to do it.tyvm for your help
